How do you draw this picture?

Hi,

I am told that I should use underlining for thoughts.
What about recall of past conversations  Italics  or underline?

I keep having these crises of confidence!

Any ideas on this one please

She recalled the conversation with Joan
“I sympathise Pippa, but sooner or later you’ll have to let go and be yourself.  Rob needs you, too.”

She looked in the mirror, her red hair unkempt from the previous night’s sleep and her bleary eyes looking like she had just awakened from a serious operation.

I've never heard of underlining text for anything. The convention is normally to use italics for internalised thoughts or dialogue unless an entire swathe of text is meant to be reflection: e.g.

Writing in third person perspective -

Pippa thought back to the conversation with Joan. "I sympathise, but sooner or later you'll have to let go and be yourself. Rob needs you too."
Easy for Joan to say - but what about her own needs?

and in first person perspective -

She looked in the mirror, her red hair unkempt and her eyes bleary.
My God, did I die in my sleep?
